Skills Instructor, Day Habilitation Program
Medicaid Regulations: Developmental Specialist

Job Summary:
The Skills Instructor has responsibility for assigned consumers and for the implementation of those consumers' individual service plans. The Skills Instructor will provide the following to assigned consumers: developmental skills training, implementing a therapeutic service plan (ISP) for all assigned consumers and complete documentation which supports such plan. S/he will work as a team member to foster an environment of staff and consumer empowerment.
Essential Functions:
- Assist consumers to identify and develop goals and plans to address any concerns, wants or needs they have.
- Provide ongoing support, guidance and role modeling for consumers.
- Works with the case manager to provide developmental skills training including training in self-care, self-preservation, receptive and expressive communication, self direction, activities of daily living, hygiene, grooming and first aid.
- Works with case manager to create and implement a comprehensive written therapeutic service plan (ISP) for all assigned consumers.
- Complete all documentation which supports ISP plan.
- Spend at least 95% of working hours in personal contact with consumers.
- Must be proactive and focus on consumer advocacy, empowerment, and community integration.
- Participate in planning and implementation of daily activity schedule and daily routines.
- Maintain certification in CPR, First Aid and SOLVE.
- Participate in staff meetings as required.
- Attend other trainings as assigned.
- Assist consumers with personal care.
- Perform all duties in accordance with the agency's policies and procedures.
- Work as a team member to foster an environment of staff and consumer empowerment.

Qualification/Education/Experience:
- Bachelor's degree in a human services related field including psychology, education, special education, recreation therapy, adaptive physical education, rehabilitation counseling, social work or sociology and one year of experience in a training program for the mentally retarded or the developmentally disabled.
- Two years of demonstrated, related experience in case management and developing and implementing teaching strategies may be substituted for a degree.
- Demonstrate strong oral and written skills (English proficiency) with consumers, families, funding sources, consultants, the community at large, program and administrative team.
- Demonstrate an ability to work as a team and perform job duties with minimal supervisory intervention.
- Must hold a valid drivers' license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
