Senior Case Manager
Reports to: Day Hab Team Leader
FLSA Status: Non-exempt
Job Summary:
The Senior Case Manager is responsible for ensuring that the highest quality support is afforded to all Day Hab members. This responsibility includes: creative program development and implementation, compliance with Day Habilitation regulations, delivery of strategies identified in therapeutic support plans, member satisfaction, and demonstrating advocacy for member self-direction. The Senior Case Manager will also assist the Team Leader with extra assignments with a larger focus on guiding the day to day operations of the program. These duties include, but are not limited to: the fulfillment of overall program development, adherence to policies and procedures, managing the program budget, materials and space, training and supporting the team, while fostering an environment which supports staff and consumer empowerment.
Essential Functions:
- Collaborate with the Team Leader to provide continuous quality improvement of individualized programs and services through the use of both internal and community resources.
- Assist team members with the development of program curriculum designed to promote individual growth and self determination. Implementation methods should include available community resources as a viable strategy to achieve individualized and program goals.
- Develop and implement comprehensive written therapeutic service plans/individual support plans and implementation process for all assigned consumers.
- Convey changes in consumer goals to the larger internal support team.
- Maintain member records as required including timely and thorough completion of necessary program assessments, progress reports and other required documentation.
- Compile review summaries for therapeutic service plans/individual support plans.
- Advocate for consumer rights; ensure that consumers are treated with dignity and respect in accordance with Advocates Human Rights Policy.
- Communicate census and any consumer needs to the daily Multi-Disciplinary Team review meeting.
- Participate in the daily Emergency Response Team (ERT).
- Ensure continuous communication between Day Habilitation staff, consumers, families, funding sources, consultants, the community, and administrative team.
- Must maintain all required trainings/certifications (First Aid/CPR, SOLVE, etc.).
- Ensure that the daily attendance/billing is complete and accurate.
- Perform all duties of the Case Manager as needed.
- Attend and actively participate in all team meetings and program trainings.
- Ensure that all consumers' personal care needs are met.
- Remain alert at all times throughout scheduled workday.
- Perform all duties in accordance with the agency's policies and procedures.

Qualification/Education/Experience:
- Bachelor's Degree in related field and 1 year of related experience or 2 years of related experience.
- Must have ability to communicate effectively in writing.
- Must have ability to read English and communicate effectively in the primary language of the program to which he/she is assigned.
- Must be able to multi-task with frequently changing priorities.
- Must possess basic computer knowledge.
- Must have a demonstrated ability to work independently and as part of a larger resource team.
- Must hold a valid drivers' license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
- Must be able to satisfactorily perform each essential function.
